Third, although high ED use has often been viewed as a proxy for a lack of access to primary care, most studies have not been able to assess the impacts of different care by different primary care providers (PCPs). Second, although some studies have used survey data to explain the Medicaid–commercial difference in ED use, there have been fewer efforts to assess the extent to which ED use could be explained by Medicaid–commercial differences in observable factors such as patient demographics, comorbidities, neighborhoods, and proximities to services to explain differences. First, although high ED use is a visible target for policy makers, less is known about whether the Medicaid–commercial utilization difference is unique to the ED or if it persists across other types of health services. There are several gaps in the knowledge of ED use among Medicaid beneficiaries. 6, 8, 9 Medicaid enrollees also might use the ED more frequently because they typically make minimal co-payments for ED visits, 1 or because they perceive the ED as a one-stop shop that provides multiple services simultaneously, an attribute appealing for patients who struggle with transportation. Medicaid enrollees have a higher comorbidity burden 1, 6, 7 and are more likely to experience primary care access problems or unsatisfactory primary care. 5 However, those policies may have limited effectiveness if they are based on incorrect assumptions about the underlying reasons for high rates of ED use in the Medicaid population.Ī variety of factors may explain differences in ED use among Medicaid and commercial patients. 1–4 State Medicaid programs have proposed different policies to reduce ED visits including requiring Medicaid patients to make higher co-payments for their ED visits or providing Medicaid patients with robust alternative services to ED care through patient-centered medical home models. T he high rate of emergency department (ED) use by Medicaid enrollees has been a long-standing concern among policy makers. The remaining unexplained difference suggests that appropriately reducing ED use remains a credible target for policy makers, although success may require knowledge about patients' perceptions and behaviors as well as social determinants of health. Models that further accounted for patient access to different primary care physicians explained an additional 8% of the Medicaid–commercial difference in ED use, suggesting that the quality of primary care may influence ED use. This suggests that relative to mental health and inpatient care, a large portion of ED use cannot be explained by administrative data. Demographics, comorbidities, health services use, and neighborhood characteristics accounted for 44% of the Medicaid–commercial difference in ED use, compared to 83% for mental health care and 75% for inpatient care. Statistical decomposition methods were applied to ED, mental health, and inpatient care using 2011–2013 Medicaid and commercial insurance claims from the Oregon All Payer All Claims database. The objective of this paper is (1) to provide context for ED service use by comparing Medicaid and commercial patients' differences across ED and non-ED health service use, and (2) to assess the extent to which Medicaid–commercial differences in ED use can be explained by observable factors in administrative data. Today, hand-drawn animation techniques are largely out of favour in the TV and film industries, with computer animation more popular. They also used flipbooks to plan drawings and in-betweens. Animation teams from Steamboat Willie right through to Who Framed Roger Rabbit drew incremental sequences of linear images onto celluloid and projected them in rapid succession. With the advent of video, flipbook principles then laid the groundwork for traditional cel animation techniques. Measured and precise, with the author’s head neatly encased within an apparatus of his own devising, the drawing presents a mechanistic depiction of both inventor and the process of invention that appears very different from the dynamic picture of a brooding genius executed by West. These permitted ambient light into the vessel interior and allowed its captain to visually chart a course without emerging from the hatch. The drawing depicts the conical glass sections inserted into the conning tower of the submarine. This 1806 painting offers a romantic picture of the forty-one year old engineer that unmistakably links the explosive activity of his torpedoes (seen here in the background) with what one of Fulton’s eulogizers would later call “the fire of his own genius.” 2 The second image offers an instance of Fulton’s own self-fashioning-a self-portrait found amid the technical drawings detailing his submarine design (fig. However, the extraordinary lengths to which he went to see the project put into practice left behind a substantial documentary record, brimming with insights into the relationship between art and invention in the early nineteenth-century. Fulton failed again and again to find a market for his work, searching first in France, then in England, and finally in the United States. Arguing that the threat of undetectable attack could finally end maritime warfare, he promoted the system as a means to secure “the liberty of the seas” and “the happiness of the earth.” 1 In spite of such optimism, the project was ultimately unsuccessful. 1)-what he called “torpedoes”-and even delivering these torpedoes directly to enemy boats via a submersible vessel and/or harpoon (fig. How common are missed miscarriages? You may find it reassuring to know that you're not alone in going through this loss. Alternatively, perhaps there were the right number of chromosomes, but a piece of one was missing or duplicated. At the moment of conception, when the sperm meets the egg, 23 chromosomes from each parent should meet, to make 46 in total. Perhaps your embryo had more or less chromosomes than is typical. The reasons for this aren’t certain but researchers believe that it’s often caused by chromosomal variations. What causes a missed miscarriage?A missed miscarriage usually happens because something went wrong in your early pregnancy. Lots of people go through a period of grief, anxiety or depression and you and your loved ones should give yourselves time to come to terms with it. The news will be a shock, particularly if you had an earlier scan that showed a heartbeat. This is when you may be told either that the pregnancy sac is empty, or that your baby has no heartbeat. Because these signs can be subtle and you won’t necessarily experience the usual miscarriage symptoms, such as cramping pain or vaginal bleeding, you may not find out that anything's wrong until your first ultrasound scan. Your breast tenderness may ease, and any nausea may stop before you expected it to. However, if your hormone levels are starting to fall, those signs may decrease slightly. This means that if you took a pregnancy test, it may still give you a positive test result for a little while. Your body may still give you signals that you're pregnant because your placenta can develop and produce a pregnancy hormone called human chorionic gonadotropin (hCG). Occasionally it happens beyond the first few weeks, perhaps at eight weeks or 10 weeks, or even further on. Or it may be that your baby started to grow, but then stopped growing and they have no heartbeat. This is called an anembryonic pregnancy, which is also known as a blighted ovum. In a missed miscarriage, it may be that your embryo didn’t develop at all and the pregnancy sac is empty. This is a missed miscarriage, also called a silent or delayed miscarriage. This means you won't immediately know that anything has happened. Your baby doesn't continue to develop but your body doesn’t pass any pregnancy tissue and you don't experience pain or bleeding. What happens in a missed miscarriage? After conception, a fertilised egg implants in your womb (uterus). Any pregnancy loss is devastating, but a missed miscarriage can also be a big shock because you may not have known anything was wrong. Unfortunately, it is often only discovered during a routine scan in the early weeks of pregnancy.
